--that's what I thought after the webinar last week as well, until I watched it again today--he actually said the CD is an extra $25 (that he thought he spent around $140 total)

Erica,

The "as indicated" language can easily be interpreted to mean "unless contraindicated". I would refer to the AOA Clinical Practice Guidelines to develop a perspective or plan.
