If you are audited by VSP due to a contact lens related issue, VSP will look to see if you have adequately documented your CL fitting. If you are missing required information your fees for fitting will be rejected and it can lead to demands to repay VSP in excess of $100,000. I developed this form to make it easy to survive a VSP CL audit by ensuring proper documentation exists for every CL encounter. I highly recommend you use some kind of distinct CL fitting form if you regularly bill VSP for CL services.
