What they’re trying to do is normalize automated refraction as a prescription as part of the on-going effort to separate optical from optometry and make it a stand-alone retail endeavor.

It’s disconcerting that the VEE administration is so stupid as a group that they’re eager to “embrace new technology” and have it at their conference rather than rejecting it outright as the f’ing poison it is.

You do understand exactly how close we are to that, right? I mean, we're essentially just a few signatures away from it and it's EASY to sell it as a public service. Hell, as much as I know how bad it is, I could get Congress on board with it tomorrow because the talking points sell themselves.

Ask them how their "innovative" technology detects ocular disease that does not immediately impact vision like glaucoma, retina detachments, diabetic retinopathy, etc.
Also ask them if they are aware that just updating a patient's Rx can delay treatment of the conditions I just mentioned.

Oh and tell them Dr Ware in Tupelo, Mississippi says they can go to hell.

I hate to be negative Gretchyn. You can try but they'll likely just paint the same rosy picture their long mission statements outline. I would make a list of points made here and perhaps by others to see how they field the questions.

The real problem is it may be what 3rd party wants in their models as well as much of the general public

Sure, ask them if they have any understanding or concern for the dramatic effect on public health of removing refraction from the ocular health portion of an eye exam OR how their system detects/accommodates for both disease processes that affect rx like corneal edema from contact lens wear or even things like accommodative dysfunction. MOST of the reason blindness in this country is so much less of a concern for Americans than other parts of the world is that:

1) A prescription requires a DOCTOR
2) A doctor is required to assess incredibly important ocular health metrics as part of the examination for that prescription.

There are many things that can affect a final prescription and the thing that makes a doctor better than an autorefractor is that we UNDERSTAND those things and are evaluating things like the cornea, retina, accommodative system etc before issuing a final prescription. We, many times, don't just write what's on the phoroptor on a piece of paper and virtually never whats on the autorefractor.

We take those subtle nuances for granted as actually "beneath us" in the world of being medical optometrists, but they're only simple because of years of training and learning how patient care actually works.

I doubt this tech is ready for prime time.

However, the trends and human nature favor this type of thing in the near future. I’m not promoting that. I’m just stating the obvious.

Payers don’t want to reimburse. Pts don’t like to pay much. Convenience reigns. Optometry enabled/endorsed crazed discounts and free care. Some ODs don’t practice to scope-level.

The tech will improve quickly.

Many ODs screen, detect, refer. Anyone can or will be able to screen, detect, refer. There’s no future in it.

We’re regulated, not owed. Hoping regulatory capture will protect optometry, a very small profession with declining advocacy membership, is unwise.

We’re capable of mx’ing these trends. I’m not sure we will do so. But we’re capable.
 
2022 at VEE, I acted aloof at the booth promoting tele-optometry. Tech does exam, doc is on screen. A sales rep told me I look familiar. I did troll comments on their FB page. Turns out rep confused me with a different doc.

We're going to take a hit at first but if we continue with superior care the retail companies will start to fizzle.

I understand I don't know how many patients are happy with remote exams but I also know that 9 out of ten patients that complain to us about their last exam all complain about the same office. And that is the office doing remote exams.

Serious question: Is there really an undersupply of ODs? I mean if the public actually needs "x" number of ODs in a given area but there are twice as many opticals in that area that can't find a doctor is there really a "shortage" or just too damn many opticals?

You have to appreciate the generational differences in play here. Most M's and even more GenZ MUCH prefer a digital option over a human. If there's not an app for it, they don't even consider it legitimate.

WE understand the limitations of remote and AI, but patients don't and if they can get an "eye exam" from their desk at work (what they perceive as an eye exam, that is - hence the need for the state boards to act as their advocate), they consider that VASTLY superior to an in-office visit.

yup. this is my concern. many of us have a hard time seeing outside our world view and opinions.

We just need to make sure we protect the patients and honestly our profession. we will adapt somehow but we cant just
let companies provide healthcare without a license

I would wager that just about every OD here has seen patients with conditions that could have led to life threatening consequences if not caught during a routine exam but hey lets just say to hell with that stuff and just update their glasses so we can make things even worse on down the road.

Too many folks seem to be easily blinded by shiny new tech stuff when in reality it is the same old bullshit that companies like Opternative have been pedaling for years. Might as well put a damn eye chart on a box of Wheaties and call it a "vision checker".
Adapting to shoddy technology is not always the answer if we truly care about the health of patients.

Perhaps it's not being a clinician that led you to think that's a valid comparison.

There's very little functional difference between Uber and a taxi. A cab driver doesn't receive more "driving classes" than an Uber or Lyft, there's just a difference in regulation and traffic laws are universal for all drivers. For things like taxi vs uber, it's just a market force issue and that's absolutely not the case here.

In this case, there's an indescribable gulf between an OD examining a patient and that patient having an online refraction. While the "market" may want to have online refraction, they're truly not educated enough to understand what they're NOT getting and that's where a little bit of regulation by a board absolutely protects them from their own ignorance. Most patients have no understanding of or appreciation for what we're actually doing outside of asking whether one or two is better.

Giving people rides does not compromise the health of their eyes and delay diagnosis of vision and even life threatening conditions. Sigh. I KNOW that Uber/Lyft is not the same as an ocular health exam. I was pointing out that in today's world, need has nothing to do with it.

Don’t roll your eyes like “these dumbasses didn’t get my point.” There’s a huge difference between two people with the exact same training and abilities doing the same thing and a retailer trying to manipulate the market to allow them to do something potentially harmful.

With Uber/Lyft, it was the market creating an equivalent, more convenient alternative. With this, it’s a retailer trying to fraudulently create the ILLUSION of an equivalent under the guise of convenience.
